Application for Instrumental Lessons

If your child wishes to take up instrumental lessons at EWS, please complete and submit this form.
You must complete this form for each child and each instrument they wish to take lessons in (therefore, one child wishing to take up two instruments would require this to be completed twice)
Request to End Instrument Lessons
Should your child no longer wish to continue with instrumental lessons, please complete and submit this form.
Please remember that a notice period of half term is reqiured, so charges will still apply until at least the next school break.
